In this capacity, you will collaborate closely with a multidisciplinary team of clinical and non-clinical professionals to implement individual and group activities tailored to rehabilitation goals. Your day-to-day involves assessing patient needs, developing personalized recovery plans, and facilitating transitions back into community life, including work, education, and independent living. You will also support specialized therapeutic interventions such as ACT, DBT, and CBT, while managing complex care coordination and linkage to essential community services.
Beyond direct patient care, the role includes maintaining program integrity through accurate documentation, participating in case conferences, and managing supply logistics. You may also assist with billing processes for Medicaid, Medicare, and private insurance, ensuring the program's financial sustainability while adhering to strict health system and regulatory standards.
