About the Role
Find your calling at Mercy as an RN II in Advanced Primary Care. This role provides virtual care to patients who benefit from ongoing chronic disease management. In partnership with a multidisciplinary team—including physicians and advanced practitioners—you will be responsible for creating patient relationships, monitoring vital sign trends, conducting patient visits between regular office visits, assisting in panel management, and providing individualized care to improve patient outcomes.
As a compassionate and skilled Registered Nurse, you will help transform chronic disease management and improve patient outcomes through innovative, patient-centered care. You will work closely with primary care clinicians in ambulatory settings to coordinate care, assessing medically complex patients and managing acute exacerbations of chronic conditions.
Location and Work Model
This position is based in a hybrid model. Following training, you must be located within 1 hour of the Chesterfield Virtual Care Office.
