About the Role
As an Admission Coordinator, you will serve as the vital link between prospective residents, their families, and our facility. You will guide individuals and families through the admission process, ensuring a smooth transition from hospital or home to our care. Your day will involve conducting facility tours, explaining our services, and managing the administrative and financial details required for admission. You will work closely with case managers, discharge planners, and social workers to coordinate transfers and ensure that all regulatory and financial requirements are met efficiently.
This position requires the mental maturity to thrive in a fast-paced environment. You will interact with persons who are ill, disabled, elderly, or emotionally upset, often managing complex behaviors and memory impairments with compassion and professionalism. Your ability to navigate these emotional dynamics while maintaining operational flow is essential to our mission of providing high-quality care.

Key Responsibilities
- check_circleConduct morning rounds to ensure vacant rooms are presentable for tours
- check_circleInitiate TARs for patients with Medi-Cal coverage on the day of admission
- check_circleTrack HMO patient length of stay and progress
- check_circleCoordinate with case managers and discharge planners to ensure smooth resident transfers
- check_circleDetermine primary and secondary payers and prepare Admission Agreements
- check_circleFollow up with family members regarding placement and discharge status
- check_circleObtain signatures on consent forms and assist with state and regulatory applications
- check_circleProvide facility tours, explain services, and distribute brochures to prospective residents
- check_circleParticipate in admissions, rehab, and quarterly quality assurance meetings
- check_circleVerify Medicare, Medi-Cal, SSI, and HMO coverage and collect necessary documentation
