About the Role
We are seeking an experienced or newly graduated Physical Therapist to join a growing team at a Skilled Nursing Facility (SNF) / Long Term Care (LTC) location in Pueblo, Colorado. This 13-week travel assignment offers the chance to explore new places while providing essential rehabilitation services.
In this role, you will assess referred patients to determine their medical conditions, functional capabilities, and rehabilitation potential. You will establish and administer treatment programs with specific goals under physician direction, ensuring realistic and achievable outcomes. Your day-to-day responsibilities include documenting patient progress, educating patients and families on physical therapy methods, and collaborating with all disciplines to evaluate and modify care plans.
