About the Role

Supplemental Health Care is seeking a Travel Home Health Physical Therapist to provide compassionate, patient-centered care in Logansport, Indiana, and the surrounding areas. This role exists to connect clinicians with top home health partners nationwide, ensuring patients receive skilled interventions in the comfort of their own homes.

In this position, you will partner with RNs, OTs, SLPs, and physicians to ensure coordinated, interdisciplinary care. Your day will involve conducting gait and balance evaluations, home safety assessments, and mobility training while educating patients and caregivers on exercise programs and safety strategies. You will document all care in compliance with Medicare and OASIS-E standards, balancing 5–6 visits per day with travel time factored into your productivity expectations.
