Key Responsibilities
- check_circleProvide care management to health plan members through an interdisciplinary approach
- check_circleOversee caseload of managed care members and ensure regulatory compliance
- check_circleComplete clear, concise, and timely documentation of care activities
- check_circleCoordinate support staff including LPNs, Community Health Workers, and Care Coordinators
- check_circleAct as a liaison and advocate for members, families, and providers
- check_circleCoordinate services and resources in collaboration with members and healthcare teams
- check_circleCreate Person-Centered Service Plans (PCSP) based on UAS-NY assessments
Requirements
- verifiedRegistered Nurse license and current registration in New York State
- verifiedAssociate degree required; BSN preferred
- verifiedMinimum one year of nursing experience
- verifiedValid NYS driver’s license
- verifiedAbility to travel in all weather conditions
