This is a multi-site role between Idylwood Care Center (Sunnyvale) and Santa Clara Post Acute (Santa Clara). As a Physical Therapist, you are the person who brings patients' power back. You will assess patient needs, develop recovery plans, and deliver physical therapy designed to restore, reinforce, and enhance physical abilities.
You are a healer and a teacher, equally adept at addressing the whole patient as an individual. You will find joy in the variety of work and settings, adapting to patient needs while collaborating with peers and making referrals to ensure patients thrive. Your role involves training patients and caregivers on the skills needed to promote independence and productivity. Reporting to the Director of Rehab, you will work in a facility environment to deliver personalized care exactly where and when it is needed most.
