About the Role

H2 Health Prime Living offers 1:1 patient care within luxury Senior Living communities in Lowry and Arvada. This role is designed for clinicians who want to see all their patients under one roof while enjoying a flexible schedule with no weekends or holidays required. You will work as part of an interdisciplinary team, collaborating to provide the best therapy opportunities for our patients. As a provider, you will serve as an extension of the communities we serve, making patient advocacy easy and impactful.
