About the Role
FOX Rehabilitation is growing in Tigard, OR, and we are looking for passionate, licensed Physical Therapists who want to deliver exceptional one-on-one care to older adults right where they live. Whether treating in private residences or within Senior Living Communities, you will help patients improve mobility, independence, and quality of life through our signature Geriatric House Calls™ model.
This position is located within one of our Senior Living Community partners. It is ideal for Physical Therapists who value autonomy, flexibility, and truly meaningful patient outcomes, while enjoying the freedom to create and manage their own treatment schedules and build strong, lasting patient relationships. FOX clinicians may also incorporate telehealth and other forms of virtual care into their schedules, allowing for greater flexibility, improved continuity of care, and the ability to support more patients.

Key Responsibilities
- check_circleProvide one-on-one physical therapy to older adults in private residences and senior living communities
- check_circlePerform patient evaluations and develop individualized plans of care
- check_circleDeliver evidence-based treatment to improve patient mobility and functional independence
- check_circleCollaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
- check_circleEducate patients, caregivers, and staff to promote safety and aging in place
- check_circleProvide telehealth and virtual care sessions to extend patient access and flexibility
- check_circleDocument patient progress effectively within the FOX EMR system in accordance with regulatory standards
Requirements
- verifiedValid Physical Therapy license in the state(s) of practice or eligibility to obtain
- verifiedDegree from an accredited Physical Therapy program
- verifiedBasic computer and EMR proficiency
- verifiedCurrent CPR certification
