About the Role
Encompass Health is seeking a dedicated Pharmacist to join our team in delivering high-quality, integrated healthcare focusing on rehabilitation and home health/hospice services. Our pharmacists play a pivotal role in our commitment to help patients regain independence after serious illnesses or injuries.
In this position, you will manage medication therapy and oversee the medication management system, ensuring a thorough understanding of pharmaceutical care practices and a steadfast commitment to patient safety. Your day-to-day impact involves assessing the appropriateness and effectiveness of patient medications, monitoring health progress to identify potential interactions or side effects, and advising the interdisciplinary healthcare team on drug dosages, storage, and safety protocols.
Beyond clinical oversight, you will assist in creating and managing medication therapy programs while educating patients on their prescribed medications, including usage, potential side effects, and interactions. Success in this role requires staying abreast of the latest drug developments and regulations while maintaining accurate records of medication inventory and patient prescriptions. You will collaborate closely with healthcare professionals, utilizing excellent communication skills to bridge the gap between complex medical information and patient understanding.
