About the Role
Encompass Health is seeking a dedicated Pharmacist to join our team in delivering high-quality, integrated healthcare focusing on rehabilitation and home health/hospice services. In this position, you will manage medication therapy and oversee the medication management system. You will assess the appropriateness and effectiveness of patient medications, monitor health progress to identify potential interactions or side effects, and advise the healthcare team on dosage, storage, and drug properties.
Beyond clinical oversight, you will assist in creating and managing medication therapy programs while educating patients on their prescribed medications, including usage, side effects, and interactions. Staying abreast of the latest drug developments and regulations is essential to maintaining accurate records of medication inventory and patient prescriptions. Success in this role requires the ability to work collaboratively within an interdisciplinary healthcare team and communicate effectively with both healthcare professionals and patients.
