The Nurse Practitioner serves as a registered professional nurse with advanced graduate education and clinical training, providing comprehensive hospice services under the supervision of a collaborating physician. This role is dedicated to delivering patient-centered care that complies with the state Nurse Practice Act, focusing on the evaluation, assessment, and management of terminal illnesses.
In this position, you will visit patients in their homes and communities to perform physical and face-to-face assessments, evaluating hospice eligibility in consultation with the interdisciplinary team. You will build upon community resources to plan and direct services that meet the unique needs of individuals and families. Your day-to-day work involves diagnosing medical conditions, developing individualized plans of care, prescribing medications, and coordinating with patients, families, and other healthcare practitioners to ensure seamless continuity of care.
