Day in the Life

As a CCM/PCM Care Coordinator, you will play a vital role in patient care coordination. Your primary responsibilities include conducting outreach calls to patients and documenting all interactions within the eClinicalWorks electronic health record system. This role requires a high level of organization and attention to detail to ensure seamless care management.
