The MDS Nurse (LVN/LPN) serves as a critical liaison between clinical care and regulatory compliance within our facility. This position is dedicated to coordinating and completing Minimum Data Set (MDS) assessments, ensuring that every resident's care journey is accurately documented in strict adherence to federal and state regulations.
In this role, you will do more than just process data; you will directly support the facility's reimbursement processes and champion quality outcomes for our residents. You will collaborate closely with interdisciplinary teams—including nursing, therapy, social services, and dietary staff—to gather comprehensive resident information and develop individualized care plans. Your work ensures that clinical documentation is precise, timely, and reflective of the actual care provided, forming the backbone of our quality improvement initiatives.
Your day will involve navigating the complexities of the RAI process and MDS 3.0 to maintain compliance with CMS guidelines. You will act as an educator and guide for staff, providing clarity on documentation standards and MDS requirements. By participating in resident care conferences and monitoring resident progress, you will help shape care strategies that truly meet the needs of our community. Additionally, you will be the primary point of contact for audits and surveys, ensuring our facility remains in good standing with the State Board of Nursing and federal agencies.
