About the Role

We are looking for a Life Care Manager to join our team in the Cambridge / Boston Territory. As a Life Care Manager, you will be a seasoned social worker or nurse providing both strategic direction and hands-on support. Your role involves coordinating medical care, assessing home safety, attending appointments, and facilitating meaningful family conversations.

You will conduct professional, objective assessments of older adult clients and their families, including psychosocial history and in-person meetings. You will develop effective relationships with clients, families, responsible parties, and care providers in all care settings. Additionally, you will create and communicate client-centered recommendations for care, resources, education, and support, while assisting in the implementation and oversight of these plans.

Your responsibilities also include providing client-centered problem-solving, ongoing support, and advocacy. You will arrange for and accompany clients to medical appointments, provide feedback to families, and ensure medical orders are followed. Furthermore, you will offer support and education to family members coping with loss, dementia, illness, and change. This role requires daily documentation of billing and interventions, as well as attendance at weekly in-person supervision with the Director of Clinical Services, monthly group supervision, staff meetings, and facility tours. You will also attend professional development programs to obtain and maintain licensure and certification, abiding by the Standards of Practice for Aging Life Care Professionals as determined by ALCA.

In accordance with the Massachusetts Pay Transparency Act, the expected pay range for this position is $55–$65 hourly. Actual compensation may vary based on factors such as licensure, years of experience, and other relevant qualifications.
