About the Role

As a Licensed Pharmacist, you will serve as a vital clinical partner in the home health setting. Your primary focus is to ensure the safe and effective use of medications for our patients by providing expert clinical medication management. You will work closely with nurses, therapists, and physicians to optimize individualized care plans, bridging the gap between prescription oversight and holistic patient care.

In this role, you will verify prescriptions, monitor for drug interactions and adverse events, and adjust therapy recommendations as needed. Beyond clinical oversight, you will play a key educational role, guiding patients and caregivers on medication use, side effects, and adherence strategies. You will also maintain accurate documentation in the EMR and actively participate in quality and safety initiatives to uphold CenterWell's high standards of excellence.
