About the Role
As an Internal Medicine Physician, you will participate as a member of the PACE Interdisciplinary Team in the care of high-risk older adults living in the community. You will act as the leader of the Clinical Team whose primary mission is to assist older adults to remain living in the community for as long as it is safe.
In this employed position, you will evaluate the needs of the participants and develop treatment plans in concert with the other members of the Interdisciplinary Team. The role involves participating in the PACE hospital rounding system as well as nursing home visits. This position is ideal for new graduates and offers a balanced schedule with Monday through Friday hours from 8 am to 4 pm, with no call requirements.

Requirements
