Signify Health, part of CVS Health®, is seeking a Full-time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. This role is designed for those with a passion for engaging with people and helping them on their journey to better health. You will bridge gaps in care by bringing flexible, comprehensive, and personalized health evaluations directly to people's front doors.

In this position, you will primarily travel up to 100% of the time across a defined coverage area, which may require overnight hotel stays, car rentals, and/or flights. Your primary workplace is the private residence of health plan members. These environments are diverse and may present a variety of conditions, including household pets, secondhand smoke, and other potential airborne allergens or irritants. You must be able to comfortably and safely perform your duties in these settings.

The visit lasts about 45 minutes, allowing time to connect one-on-one and answer health questions. You will conduct a medication and medical history review, a physical evaluation, and, if ordered by the person's health plan, additional diagnostic tests (i.e., diabetic eye exams, spirometry, etc.). You'll use an iPad and an intuitive clinical workflow for each evaluation. Notably, you are not required to prescribe medicine, order lab tests, or alter people's current treatment regimen.

Our concierge-style Network Activation team will support you in obtaining, renewing, enrolling, or verifying additional state licensure in new states as business needs exist.
