Key Responsibilities
- check_circlePerform and complete physical assessments for hospice Face-to-Face encounters within CMS timeframes
- check_circleDocument clinical findings accurately and attest F2F encounters in patient medical records
- check_circleUtilize Local Coverage Determination criteria per hospice policy and regulatory requirements
- check_circleCommunicate clinical findings, treatment plans, and lab results with certifying physicians
- check_circleProvide clinical recommendations to the interdisciplinary group based on F2F assessment findings
- check_circleNotify the hospice agency promptly if unable to complete scheduled F2F visits
- check_circleCollaborate with physicians, interdisciplinary teams, patients, caregivers, and families
- check_circleConduct assessments in patient residences including homes, skilled nursing facilities, or other settings
Requirements
- verifiedMaster's or higher degree in Nursing
- verifiedCompletion of an accredited graduate-level nurse practitioner education program
- verifiedCurrent registered nurse license in state of practice
- verifiedCurrent advanced practice nurse license or certification
- verifiedNational Nurse Practitioner certification (ANCC or AANP)
- verifiedCollaborative agreement in place as required
- verifiedCurrent CPR certification
- verifiedValid driver's license and automobile insurance

Experience in acute care, hospital, oncology, home health, skilled nursing, or hospice settings
