About the Role
As a Home Health Registered Nurse, you will be the cornerstone of a patient's care journey. This role exists to bridge the gap between clinical expertise and home-based support, ensuring that every patient receives personalized attention. You will conduct comprehensive assessments, develop individualized care plans, and administer critical treatments, including wound care and IV therapy.
Your day will involve a dynamic blend of clinical practice and collaboration. You will educate patients and their families on self-care and disease management, ensuring they feel empowered and safe. Working closely with physicians, therapists, and interdisciplinary teams, you will coordinate care seamlessly, monitor patient status, and maintain accurate, timely documentation.
