About the Role
As a Home Health Physical Therapist, you will report to the Clinical Manager and play a vital role in the patient care team. You will work onsite in patients' homes, planning and administering prescribed physical therapy treatments to help them attain the highest level of physical function. By assessing and documenting patients' physical strengths, you will support physician evaluations and guide treatment goals, developing and implementing rehabilitation programs consistent with the physician's Plan of Treatment and the patient team's goals.
Collaboration and Documentation
Effective communication and thorough documentation are key to your success in this role. You will provide Physical Therapy Assistants (PTAs) and Home Health Aides with clear written care instructions, overseeing their performance and documentation. You are expected to document patient observations, interventions, and evaluations promptly and thoroughly. Additionally, you will report patient progress to the physician, Clinical Manager, care team, patient, and family, submitting all required evaluations, plans, progress notes, and discharge summaries.

This role is considered patient-facing and is part of Humana's Tuberculosis (TB) screening program; if selected, you will be required to be screened for TB. Furthermore, as this role is part of Humana's driver safety program, you must have a valid state driver's license.

Requirements
