The Health Guide serves as a high-touch, customer-centered liaison between patients, the health care system, and community resources. This role exists to support patients with complex healthcare needs throughout their journey, fostering trust and developing self-management skills through advocacy, education, and proactive outreach.
In this position, you will act as a passionate advocate for health and wellness, collaborating with a multidisciplinary team to drive prevention and health promotion. The day-to-day involves navigating the intricacies of the healthcare system, arranging further care, and maintaining strict patient privacy. You will also participate in outreach calls, document progress, and assist in analyzing data to identify barriers and recommend quality improvement alternatives.
