About the Role
This hybrid position invites you to ignite your career as a Digital Health Specialist at our CORA Riverside clinic in Jacksonville, FL. You will split your time working three days in-person at the clinic and two days remotely, delivering digital health services that include virtual care and remote therapeutic monitoring.
As a Digital Health Specialist, you serve as a vital liaison between patients and in-clinic treating providers. You will participate in the development of our digital health program, utilizing a digital platform to deliver telehealth visits for evaluation, assessment, and treatment plan development. Your day-to-day involves performing Telehealth and Remote Therapeutic Monitoring (RTM) services, including interactive communication and monitoring, while working closely with operations to meet patient care and staffing needs.
You will make a powerful impact on the local community by developing and delivering personalized plans of care for a diverse patient population with both orthopedic and neurologic diagnoses. Using cutting-edge software, you will objectively measure patient outcomes and efficiently document evaluations, treatments, re-evaluations, and discharge notes. Additionally, you will maintain regular reporting and data analysis of regional service metrics and support marketing efforts to optimize and grow remote services at CORA.
Hiring Process
Candidates must be located in Jacksonville, FL (Riverside) or within driving distance. This is a full-time, hybrid role (40 hours/week, 5 days/week). Candidates must also be willing to obtain Physical Therapy licensure in other states as needed.
