About the Role
As a Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) with GuideStar Eldercare, you will provide neurobehavioral health services to residents in long-term care facilities. Using a multi-disciplinary approach, you will partner with specialty clinicians to provide holistic, continuity of care for elderly individuals who are afflicted with neurocognitive disorders and other neurologic and/or psychiatric conditions.
This role is designed for professionals with a passion for geriatrics and neurobehavioral science who desire to make a tangible difference in the lives of a population struggling with dementia and other neurodegenerative disorders. You will function autonomously with regional support, engaging in condition-specific support therapy and insight-based therapy as indicated, while coordinating care plans with facility staff, primary care physicians, and families.

Requirements
- verifiedMaster's degree in Mental Health Counseling or Social Work
- verifiedCurrently licensed as an LMHC or LCSW in the state of application
- verifiedAbility to work as part of an interdisciplinary treatment team

- verifiedAble to function autonomously with regional Area Manager support

Nice to Have
Passion for geriatrics. Experience in inpatient settings such as extended care facilities. Experience with brief bedside assessment screens (cognitive, mood, etc.). Motivated by productivity-based positions. Ability to navigate and document on an Electronic Medical Record (EMR) system.
