The Clinical Care Leader serves as the guiding force for our interdisciplinary teams, ensuring the delivery of safe, high-quality, and patient-centered care. This role exists to coordinate daily clinical operations while championing a culture of continuous improvement and professional growth. You will be the bridge between clinical data and compassionate action, using EMR insights to drive quality initiatives and close care gaps.
In this position, you will mentor staff in best practices, foster collaborative teamwork across nursing, behavioral health, and community partners, and model our commitment to trauma-informed care. Your day-to-day involves overseeing patient throughput, supporting complex care coordination, and ensuring adherence to regulatory standards, all while reinforcing the mission that places the patient at the center of every decision.
