About the Role
We are seeking dedicated Care Managers to provide support for care management and care coordination activities. In this role, you will collaborate with a multidisciplinary team to coordinate the integrated delivery of member care across the continuum. Your primary goal is to ensure member progress toward desired outcomes while contributing to the overarching strategy of providing quality and cost-effective care.

Candidates must reside in one of the following Florida regions:
- Region A: Escambia, Santa Rosa, Washington, Gadsden, Leon, Bay, Okaloosa, Walton, Wakulla, Jackson, Jefferson, Calhoun, Franklin, Gulf, Holmes, Liberty, Madison, Taylor
- Region B: Duval, Hernando, Lake, Marion, Volusia, Alachua, Columbia, St. Johns, Flagler, Citrus, Suwannee, Baker, Bradford, Clay, Dizie, Gilchrist, Hamilton, Lafayette, Levy, Nassau, Putnam, Sumter, Union
- Region C: Pasco, Pinellas
- Region D: Hardee, Highlands, Hillsborough, Manatee, Polk (highest volumes in Hillsborough, Manatee, and Polk)
- Region E: Seminole, Orange, Osceola, Brevard
- Region F: Charlotte, Collier, Desoto, Glades, Hendry, Lee, Sarasota (highest volumes in Collier, Lee, and Hendry)
- Region G: Indian River, Martin, Okeechobee, Palm Beach, St. Lucie (highest volumes in Palm Beach, St. Lucie, Indian River, and Martin)
- Region H: Broward
- Region I: Miami-Dade, Monroe
Travel Requirements
This position requires 25-40% estimated local travel, based on state and contractual requirements. You must have a valid driver's license, reliable transportation, and adequate auto insurance.

Requirements
- verified2+ years experience in health care, preferably in care management, or experience in a medical and/or behavioral health setting, or equivalent combination of relevant education and experience.

- verifiedValid and unrestricted driver's license, reliable transportation, and adequate auto insurance for job related travel requirements, unless otherwise required by law.
