The Home Health Admissions Coordinator (Intake Coordinator) serves as the vital link between referral sources and clinical care, managing the end-to-end referral-to-admission process for home health patients. This position exists to ensure that patients are admitted efficiently while strictly adhering to Medicare and payer guidelines.
In this role, you will coordinate closely with Business Development Specialists, physicians, clinical teams, and patients. Your day involves confirming referral information, obtaining necessary documentation, verifying insurance eligibility, and scheduling Start-of-Care (SOC) visits. You play a critical part in maintaining a compliant and streamlined intake workflow, ensuring that no patient is delayed due to administrative gaps.

Healthcare administration or related education. Home health EMR experience. Familiarity with Homecare Homebase (HCHB) or similar home health documentation systems. Knowledge of Medicare home health regulations, physician orders, and face-to-face documentation requirements. Experience coordinating with home health nurses, therapists, social workers, and referral partners.
