As a Case Manager RN, you will join an organization that invests in its team members. You will partner with physicians, nurses, and department leaders to ensure safe transitions, clear communication, and consistent quality across the continuum of care. Your day will involve guiding patients and families through program orientation, explaining rehabilitation philosophies and insurance benefits, and developing individualized treatment plans that honor personal recovery goals. By collaborating with the Rehab Program Director and interdisciplinary teams, you will promote HCA Healthcare's values of compassion, respect, and excellence in every interaction.
