As a Case Manager RN, you will join an organization that invests in its team members. In this role, you will support patients and families through every step of their care journey, coordinating services and connecting resources to develop care plans that reflect each patient's unique needs.
You will partner with physicians, nurses, and department leaders to ensure safe transitions, clear communication, and consistent quality across the continuum of care. Your day will involve guiding patients and families through program orientation, explaining rehabilitation philosophies and insurance benefits, and developing individualized treatment plans that honor personal recovery goals. By collaborating with the Rehab Program Director and interdisciplinary teams, you will promote HCA Healthcare's values of compassion, respect, and excellence in every interaction.
