The Behavioral Health Outreach Supervisor serves as a vital leader within the Sonoma County Indian Health Project (SCIHP), providing oversight of community-based outreach, care coordination, and substance use disorder (SUD) programs. This role is dedicated to serving Native American and SCIHP communities with a focus on culturally responsive engagement and service delivery.
The Supervisor ensures the effective management of the outreach team and SUD counselors, bridging the gap between individuals and essential mental health, substance use, recovery, and supportive services. A key component of this position is connecting community members to safe, confidential resources for domestic violence and intimate partner violence (DV/IPV) while collaborating with Tribal programs, healthcare providers, and partner agencies to reduce barriers to care and promote stigma reduction.
This role blends high-level leadership with direct community engagement. You will foster a trauma-informed, inclusive work environment that supports staff wellness and professional development while managing caseloads and program implementation. Beyond supervision, you will represent the program at Tribal events, community meetings, and public forums, delivering presentations on mental health awareness, prevention, and harm reduction strategies.
You will also maintain a care management caseload, conducting assessments and developing individualized care plans. The position requires extensive collaboration across internal departments and external partners to coordinate the full SUD continuum, from prevention to recovery support. Additionally, you will facilitate warm hand-offs to culturally responsive services, assist participants with insurance navigation, and provide technical assistance to Tribal communities on crisis intervention and behavioral health best practices.
