Amwell Medical Group (AMG), in partnership with Amwell, is a cutting-edge telehealth clinical practice. We are seeking board certified family medicine and internal medicine physicians to join our Virtual Primary Care program. This program will focus on clinicians and patients establishing a longitudinal relationship for more comprehensive and informed care, leveraging the power of a digital telehealth platform to ensure the highest quality outcomes. Clinicians are required to commit 10 hours per week, at times that are preferable to them, within the program hours of Monday-Friday 8am-8pm. Our virtual primary care program's focus is on achieving healthy outcomes over time. Patients will schedule visits with their AMG clinician for initial intake and assessment, as well as focused visits for established chronic conditions, general wellness and screening exams, medication management and follow-up, or same-day visits. _Your Role:_ Virtual Primary Care: Serve as the coordinating primary clinician directing the care for adults and children 12 years of age, covering a variety of wellness needs and acute and chronic conditions including asthma, hypertension, diabetes, COPD and more. Refer patients for specialty care when indicated, leveraging local hospital systems and outpatient testing centers. Coordinate care with in-person providers when necessary. Prescribe DME and diagnostic/monitoring tools (BP cuff, peak flow meters) as indicated. Using a DocuSign workflow, complete forms as needed for patient support in the community. Order and review labs through an integrated portal directly from the chart, messaging patients with results or following up in a visit when appropriate. Send and receive secure messages with patients and nursing staff who will monitor and assist with messaging for providers. Ensure ample visit lengths to address needs in a thorough visit supported with digital data gathering and tracking, gaps in care check-ins (VPC), post visit monitoring for some conditions and substantial nursing and technical support. _Requirements:_ Minimum 1 active state license Active board certification in internal or family medicine Minimum 2-year post-residency experience At least one active, unrestricted DEA license Job Type: Contract Expected hours: No less than 10 per week Benefits: Flexible schedule Schedule: Choose your own hours Monday to Friday License/Certification: State License (Preferred) unrestricted DEA license (Preferred) Board Certification in IM or FM (Preferred) Work Location: Remote
