Overview We are seeking a skilled and compassionate Physical Therapist to join our home health team, focused on providing personalized care to adult patients in their homes. The ideal candidate will have a passion for helping patients regain their mobility and independence, possess a strong knowledge of anatomy and medical terminology, and be dedicated to delivering high-quality patient-centered care. This role offers the unique opportunity to work in patients' homes, providing one-on-one care tailored to their individual needs. Duties Conduct in-home assessments and develop individualized treatment plans for adult patients. Implement therapeutic exercises and interventions to aid in patient recovery and improve mobility and independence. Monitor patient progress regularly and modify treatment plans as necessary to achieve optimal outcomes. Collaborate with physicians, nurses, and other healthcare professionals to ensure comprehensive, holistic care. Provide education and guidance to patients and their families on home exercise programs, injury prevention, and strategies to maintain physical health. Utilize a variety of therapeutic modalities and techniques to address pain management, strength training, and functional mobility. Ensure proper documentation of patient care, progress notes, and treatment plans in compliance with regulatory and company standards. Qualifications Degree in Physical Therapy from an accredited program. Valid state licensure as a Physical Therapist.
