JOB SUMMARY We are seeking a highly motivated Licensed Independent Social Worker (LISW) with a passion for treating complex psychiatric conditions and improving the quality of life for residents in a long-term care setting. As a Psychiatric Care Coordinator, you will play a pivotal role in coordinating psychiatric care for our patients, collaborating closely with multidisciplinary teams to address their mental health needs. PRIMARY RESPONSIBILITIES AND ESSENTIAL FUNCTIONS Duties of all PHP Licensed Independent Social Workers Coordinate Psychiatric Care: Work closely with residents, their families, and healthcare providers to assess psychiatric needs and develop individualized care plans. Multidisciplinary Collaboration: Collaborate with physicians, nurses, therapists, and other healthcare professionals to ensure comprehensive and integrated care for residents. Assessment and Evaluation: Conduct psychosocial assessments and ongoing evaluations to monitor residents' mental health status and progress. Treatment Planning: Develop and implement individualized treatment plans tailored to meet the unique needs and preferences of each resident, including counseling, therapy, and medication management. Crisis Intervention: Provide immediate support and intervention during psychiatric emergencies or crises, collaborating with the appropriate resources to ensure resident safety and well-being. Education and Support: Offer education and support to residents, their families, and nursing staff regarding the management of mental health issues. Advocacy: Serve as an advocate for residents, ensuring their rights are respected, and their voices are heard in the decision-making process regarding their psychiatric care. Quality Improvement: Identify areas for improvement in psychiatric care delivery and participate in quality improvement initiatives to enhance the overall quality of life for residents. Documentation and Reporting: Maintain accurate and thorough documentation of assessments, care plans, interventions, and outcomes in compliance with regulatory standards. Professional Development: Stay informed about current trends, best practices, and evidence-based interventions in the field of psychiatric social work through ongoing education and professional development activities. EDUCATION and QUALIFICATION Required: Master's degree in Social Work from an accredited institution. Current licensure as a Licensed Social Worker (LPC, LPCC, LPCC-S, LSW, LISW, or LISW-S) in the state of Ohio. Strong clinical assessment and diagnostic skills, with the ability to effectively evaluate and address mental health concerns in older adults. Excellent communication and interpersonal skills, with the ability to collaborate effectively with diverse stakeholders. Proven ability to work autonomously as well as part of a multidisciplinary team. Commitment to ethical practice and adherence to professional standards of conduct. Proficiency in computer skills for documentation and communication purposes. Compassionate, empathetic, and patient-centered approach to care.
