Overview Responsible for coordination and implementation of in-house programming, including assessments, goal planning, evaluation of daily progress, monthly resident reviews, monitoring recreational programs, seeking community resources, and maintaining a trip log for van use. Making independent judgments using discretion for the benefit of the individual, they will ensure the individuals receive the highest standard of services required and their day-to-day needs are met. All services must be followed as written in the individual’s plans. The Assistant Program Manager must be able to travel, possess good writing skills, articulate well, and be an aggressive advocate. They should function at full capacity within the duties outlined in the job description. The agency has embraced a fully integrated system of care for all individuals served, requiring all staff to assess the individual's needs holistically and connect individuals with necessary services, both internally and externally. Assess the needs of the whole individual/family, including basic needs such as food, clothing, and shelter. Ensure that referrals are made to appropriate internal and external services as required to meet the individual's or family’s needs. Follow up on referrals made on behalf of individuals or families and respond to all referrals made to the program. Participate in cross-program service reviews that support the planning and coordination of service delivery and service closure, along with other duties as assigned. Responsibilities Responsible for organization of records and ensuring records are up to date. Participate in Interdisciplinary Team meetings and staff meetings. In conjunction with the Program Manager, create and plan agendas for meetings, including committee reports, announcements, and staff concerns. Collaborate with consultants, especially psychologists, on programming. Follow through on recommendations from the U.R. committee. Attend U.R. meetings when delegated. Complete and maintain investigation certifications related to incidents within the DDS. Ensure all life safety safeguards for individuals are implemented and followed. Maintain and submit timely and accurate records and reports regarding the health and safety of individuals and the quality of each program. Submit, update, and close all incidents for the program. Maintain compliance with OPWDD and the Justice Center regarding incidents. Make records available for review for IPR&UR. Be aware of and sensitive to the cultural and socioeconomic characteristics of the persons and populations served. Prepare staff schedules for Program Specialists, including responsibilities for vacation, personal, and sick leaves, and maintaining coverage during emergency staff shortages. Ensure appropriate staffing patterns as per facility policy. Conduct scheduled supervisor meetings to discuss programmatic and staffing issues. Evaluate individual work performance of all staff through observations and performance counseling in conjunction with the Program Manager. Ensure that each individual is provided and/or assisted in skill development in areas such as activities of daily living, domestic living, recreation, and socialization as needed. Assist the Program Manager in devising and executing relevant agency/program policies, in accordance with OPWDD and/or other funding agency guidelines. Conduct frequent rounds on all shifts and submit reports to the Program Manager to ensure safety and health requirements are observed, the physical environment is comfortable and stimulating, and quality individual care and programming are provided. Perform responsibilities of a medical assistant, including tracking appointments, communicating with the CMS, nurse, relevant doctors, and managers, ensuring accuracy of medication inventory, records, and orders for refills when necessary, tracking and following up on medical appointments, completing medical referral forms, and completing monthly medical book reviews. Ensure coordination by supervisors of all transportation activities to assure maximum use of transportation services for day programming, recreation services, medical, and special requirements. Ensure the transportation of individuals to and from programs, community inclusion activities, medical appointments, home visits, and all other activities as needed. Ensure completion of the Staff Information Entry Form when escorting individuals on medical appointments. Ensure the timely receipt and ongoing availability of all individuals’ needs (toiletries, clothing, and personal effects). Prepare Day Program progress reports at ITT reviews as assigned. Distribute petty cash funds to staff or collect petty cash receipts and submit them to the Program Manager. Conduct monthly fire evacuation drills including all staff members on all shifts. Participate in admission and discharge reviews and make recommendations to the Program Manager. Develop a monthly recreation schedule in consultation with the Program Manager. Implement and monitor in-house and community recreational activities, providing hands-on assistance as needed. Other duties as assigned. Direct care as necessary. Attendance standards: 1:00 PM – 9:00 PM, Sunday through Thursday, at the discretion of the Program Manager. Qualifications High school diploma with one (1) year of experience in Human Services (Social Work, Psychology, Special Ed., etc.) or a related field required; Associate’s Degree preferred. 1-year administrative experience. 2 years minimum experience working with individuals diagnosed with intellectual disabilities (ID). Respect and consideration for the resident being served. AMAP, SCIP-R, and First Aid/CPR Certified. A valid driver’s license is required (must be able to drive a large vehicle which consists of a wheelchair or 15-seater van). J-18808-Ljbffr
