Our onsite medical center provides personalized routine and wellness care, sick visits, and specialized nursing care exclusively to residents across assisted living, memory care, and skilled nursing levels, as well as at home. We are a patient-centered team that cares for the whole person, offering a secure, supportive workplace where you can work closely with residents and their families to help them excel and grow.

About the Role

We are seeking Per Diem and Part Time Registered Nurses to join our family. In this role, you will develop relationships with residents and their families using a consistent caregiver approach, supporting their physical, spiritual, emotional, and clinical needs while placing the resident first. You will be instrumental in promoting resident independence by establishing care goals and teaching residents and families about conditions, medications, and self-care skills.

Your day will involve monitoring clinical conditions to identify changes in status, ensuring patient comfort and safety, and working alongside care associates to perform routine nursing care. You will collaborate with the interdisciplinary and neighborhood team to develop and coordinate comprehensive Holistic plans of care, ensuring that documentation is current, accurate, and timely.
