About the Role
Encompass Health is seeking a dedicated Pharmacist to join our healthcare team, focusing on providing top-notch pharmaceutical care in rehabilitation, home health, and hospice settings. In this role, you will be instrumental in helping patients regain independence after serious illness or injury by managing medication therapy and improving overall patient outcomes.
This position requires a detail-oriented individual who can work collaboratively with other healthcare professionals to ensure the highest standards of care. You will serve as a vital link between physicians, patients, and the broader care team, ensuring that medication plans are not only legally sound but also optimized for individual patient needs.
Day in the Life
Your work will involve reviewing and executing physician prescriptions with a keen eye for appropriateness and legality. You will organize the pharmacy to ensure efficient identification and delivery of products, maintaining full control over stocking and labeling processes. Beyond dispensing, you will provide specialized services to help patients manage chronic conditions such as diabetes, asthma, smoking cessation, and high blood pressure.
A significant portion of your day will be dedicated to advising patients on the correct administration of drugs, assessing treatment results, and educating them on potential drug interactions and side effects. You will collaborate closely with the healthcare team to ensure optimal patient care while keeping accurate documentation of medication plans and patient progress.
