The Pediatric Care Management Social Worker (PRN) serves as a vital link within the OU Health Inpatient Department, dedicated to managing care for high-risk pediatric members with chronic behavioral and health conditions. This role exists to ensure timely access to necessary care, continuity across all settings, and shared decision-making for patients and their families. Working under the guidance of a Social Worker II, you will provide essential support and guidance as patients navigate wellness within the OU Health system.
In this position, you will collaborate closely with an interdisciplinary care team, including physicians, caregivers, and community resources, to coordinate patient care across the continuum. Your day-to-day involves assessing unique patient conditions through data synthesis and personal interviews, developing care plans, and acting as a liaison between clinical staff and families. You will also lead care management team meetings, participate in interdisciplinary rounds, and mentor newly hired staff or social work interns.
This is a PRN (as needed) position requiring a minimum commitment of 2 shifts per month. Current OU Health employees should log in via the internal portal to apply.
