As a Pediatric Care Management Social Worker at OU Health, you will provide essential support and guidance to patients and their families as they navigate the complexities of the healthcare system. This part-time, weekend position (24 hours per week) focuses on comprehensive care coordination for individuals and families facing challenging social and health-related issues. You will work collaboratively with clients, families, healthcare professionals, and community resources to address unique needs and improve overall well-being.
In this role, you will conduct psychosocial assessments, develop care plans, and act as a vital liaison between patients and clinical staff. Your work involves advocating for patient concerns, providing crisis counseling, and offering education on diseases, financial situations, and emotional stress. You will also serve as a mentor, precepting newly hired social workers and students under the supervision of experienced team members.
