The Pediatric Care Management RN serves as a vital coordinator within the Inpatient Department at Children's Hospital. This role is designed to ensure the efficient utilization of healthcare resources while promoting quality outcomes and patient satisfaction. You will be accountable for assessing, planning, implementing, coordinating, monitoring, and evaluating the care options required to meet the unique healthcare needs of our pediatric patient population.
In this position, you will collaborate closely with an interdisciplinary healthcare team, patients, and their families to develop individualized care plans that ensure continuity of care. Your day-to-day impact involves providing education on health conditions, treatment plans, and self-care strategies, while acting as a liaison between patients, families, and community resources. You will also evaluate healthcare utilization patterns to identify opportunities for improving efficiency and cost-effectiveness, advocating for appropriate resource allocation while adhering to regulatory guidelines.
This position is scheduled Monday through Friday from 8:00 AM to 4:30 PM, located on the 9th Floor of the Inpatient unit at Children's Hospital. Please note that this role may be filled as a Level 1, 2, or 3, depending on your specific education, experience, and license requirements.
