As a Matrix provider, you will conduct comprehensive health assessments that provide unmatched insight into a member's overall health and well-being—insight that is often difficult to capture during routine office visits. During a visit lasting up to one hour, you will review and observe a member's current health, medical history, medication adherence, social environment, and other risks. This approach allows us to potentially close multiple care gaps with a single visit, improving the quality of care for our members.
You will work directly at the member's place of residence, which may be their home or a Skilled Nursing Facility. Your day involves collaborating with Primary Care Physicians, case managers, social workers, family members, and key caregivers to ensure seamless patient education and follow-up. You will serve as a vital link in the care continuum, conducting adult and geriatric assessments, administering point-of-care and lab-based testing, and providing clear documentation according to Matrix guidelines.
We offer a unique opportunity to leverage your expertise while enjoying a flexible schedule that fits your life. Visits are scheduled based on your availability, with assessments conducted Sunday through Saturday from 8 am to 7 pm. You can choose to work during the day, evening, and/or weekend. We invest in your growth through comprehensive onboarding training, 1:1 feedback from a preceptor, and free access to continuing education. Additionally, we support business needs by assisting with obtaining additional state licensure and credentialing in neighboring states.
