Job Overview: PCIL is seeking a highly motivated and organized DDD Support Coordination Supervisor to oversee and manage support coordination services under the New Jersey Division of Developmental Disabilities (DDD). The supervisor will ensure that high-quality, person-centered services are provided to individuals with disabilities, helping them access necessary resources and live independently in their communities. Key Responsibilities: Quality Assurance : Ensure that support coordination services, including individual service plans (ISPs), assessments, and case notes, comply with DDD guidelines and regulations. Case Management Oversight : Review and approve ISPs to ensure goals align with individuals’ needs and aspirations and provide ongoing monitoring of service quality. Collaboration&Advocacy : Collaborate with families, caregivers, and service providers to ensure participants receive the services and supports needed. Advocate for individuals in navigating the DDD system. Data&Reporting : Maintain accurate records, prepare reports for internal and external stakeholders, and oversee data entry in relevant databases. Professional Development : Stay current with trends and changes in DDD regulations, support coordination practices, and disability advocacy. Qualifications: Education : Bachelor’s degree in social work, Human Services, Psychology, or a related field. Master’s degree preferred. Experience : Minimum 3-5 years of experience in the field of developmental disabilities, with at least two years of supervisory experience. Certifications : DDD Support Coordination certification required or ability to obtain certification upon hire. Skills : Strong leadership, organizational, and communication skills. Proficiency in Microsoft Office and data management systems. Knowledge : Understanding of New Jersey’s DDD system, Medicaid, and community resources for individuals with disabilities.
