The Manager Case Management department provides operational leadership to the case management department by developing, interpreting and applying policies, case management criteria and strategies to ensure fiscally responsible care management of patients. Adheres to National Patient Safety Goals as appropriate based on the level of patient contact this position requires. INTEGRIS Health is an Equal Opportunity/Affirmative Action Employer. Participates in treatment planning with physician and related medical disciplines to provide the highest quality of care in the least restrictive environment to include daily monitoring of Care Management Staff, reviewing patients and progress including admission, continued stay, and discharge criteria. Chairs weekly Interdisciplinary Outlier Committee, and communicates with physicians, Case Management Director, and Administration about patient needs. Manages hospital Utilization Management Program by coordinating Utilization Plan changes with Medical Director and administration, co-chairing Utilization Management Committee, and collecting and reporting Performance Improvement data to Performance Improvement Committee and Utilization Management Committee. Ensures department staff are managed effectively, including hiring, training, evaluating performance and competency, and conducting disciplinary/counseling sessions as needed. Applies state, federal and other regulatory agencies functions and standards. RN Oklahoma license or Licensed Clinical Social Worker. Bachelor's degree in nursing preferred. Previous work experience in hospital case management preferred Previous work experience utilizing nationally recognized Level of Care Criteria Sets preferred Diabetic Educator certification preferred Must be able to communicate effectively in English (verbal/written)
