Job Details Position - RN Case Manager Location - Fort Myers, Florida, 33912 Shift - Day Shift Shift Note - 40 hours per week 8 hours per day Contract Duration - 13 Weeks Duties Conduct comprehensive patient assessments to evaluate health status and needs. Develop individualized care plans in collaboration with interdisciplinary teams. Monitor patient progress and adjust care plans as necessary to ensure optimal outcomes. Provide education and support to patients and their families regarding treatment options and health management. Maintain accurate documentation of patient interactions, assessments, and care plans in compliance with HIPAA regulations. Utilize medical terminology effectively to communicate with healthcare professionals and document patient information. Coordinate inpatient care, including discharge planning and follow-up services. Stay informed about current best practices in nursing, particularly in pediatrics and other relevant specialties. Experience Current Registered Nurse (RN) license required. Previous experience in case management or a related nursing field is preferred. Strong knowledge of physiology, vital signs assessment, and medical terminology is essential. Familiarity with HIPAA regulations and their application in patient care settings. Experience working with pediatric patients is a plus but not mandatory. Excellent communication skills, both verbal and written, with the ability to work collaboratively within a team environment. 2 years of Case Manager experience required acute care experience is required. PRIOR TRAVEL EXPERIENCE REQUIRED. MUST HAVE EPIC EXPERIENCE. BLS required. Shift: 8Hr Days, Monday-Friday 8a-4:30p. Will float to different units and may need to float to another hospital within Lee Health if there is a greater need.
