We are committed to serving the whole you Visit our website Infectious Disease Nurse Practitioner performs assessments and evaluations, and provides the appropriate information regarding specialist services, counseling and patient education for people living with HIV and other chronic infectious diseases. This role functions as a part of a large, integrated care team with the goal of providing whole-person care in the outpatient setting. Our team of primary and specialty care providers work in collaboration with patients and caregivers to address both the body and mind. Our practice includes primary and chronic disease management with a particular focus on individuals living with HIV and transgender care. About the Role: Manage and improve health for patients with multiple chronic medical conditions. Work in a team environment (with Nursing, Medical Assistant, Social Work, Behavioral Health, Pharmacy, Nutrition, etc.) to process feedback and be open to discussing and implementing plans of care from other members of the team. Work well with a supervising physician. Develop strong transitional care plans and track paneled patients along the whole continuum of care including discharges from the hospital, ER, skilled nursing facilities/nursing homes, and home care. Treatment of acute exacerbations of chronic conditions. Accept same day/next day or urgent appointments for patients. Track patients and their disease processes to prevent ER visits, admissions to the hospital, and readmissions. Understand the PCMH model of care as it pertains to specialist care, only utilizing this higher level of care as needed, and not as a substitute for primary care using evidence-based clinical skills. Educate patients regarding their chronic diseases, healthy choices, chronic disease progression, and advanced directives. Practice evidence-based medicine in accordance with the PCMH model of care. Track and successfully achieve specified quality metric goals, ensuring preventive care measures are addressed in an effective and timely manner. Use technology and the EMR to the highest capacity to increase quality initiatives and efficiency within the medical center. Participate in multi-disciplinary meetings and recommend treatment alternatives designed to keep patients healthy or resolve ongoing clinical issues. About You: Georgia Licensed Nurse Practitioner or Physician Assistant. Experience with patients who have multiple chronic medical conditions, i.e., HIV, Diabetes, HTN, COPD/Asthma, CHF, Chronic Renal Failure. American Heart Association BLS certification. Experience with management of Medicare, Medicaid, and Commercial populations. Proficiency with EMR, eClinicalWorks preferred with notes completed within 48 hours. Knowledge of chronic medical conditions, i.e., HIV, Diabetes, HTN, COPD/Asthma, CHF, Chronic Renal Failure. Knowledge of Medicare, Medicaid, and Commercial populations. Excellent organization and communication skills. Strong interest and background in promoting sexual health and serving LGBTQ communities.
