Location: CHA Cambridge Hospital Work Days: Mon-Fri; 8:00-4:30, rotating weekends and holidays Category: Human and Social Services Department: Inpatient Case Management CH Job Type: Full Time Work Shift: Day Hours/Week: 40.00 Union Name: Non Union Summary: Responsible for working as part of a multidisciplinary team on the inpatient medical, ICU and ED units executing elements of the discharge plan, arranging and facilitating team/patient/family meetings and providing resources while under the supervision of licensed Care Management staff. Develops awareness of community resources to meet the needs of patients based on treatment factors and population health. Minimum Requirements: Both prior experience working in a hospital setting and bilingual skills is a plus. Experience using electronic medical records is necessary; administrative experience desired. Weekend rotation duties. Demonstrated experience working in multicultural and multidisciplinary settings required. Cognitive ability: Ability to multitask and exceptional organizational skills required.
