We are seeking a quality-driven Nurse Practitioner or Physician Assistant to join our post-acute care team in Asheville, North Carolina. This full-time role (4 days/week) focuses on rounding in skilled nursing facilities, providing you with significant autonomy and the opportunity to build meaningful relationships with patients and their families. You will collaborate with an experienced multidisciplinary team to deliver exceptional care, utilizing innovative population health data reports to guide optimal and timely treatment decisions.
This position is designed for those who want to make a tangible impact in post-acute care. You will work within a supportive framework that includes a comprehensive 3-month onboarding program, a dedicated clinical field mentor, and access to the Medical Director Leadership Academy (MDLA). This best-in-class educational program positions you for success in quality improvement initiatives and fosters collaboration with facility leadership as a valued partner.
